Support for Effective Disease Self-Management
NW GWEC Partners worked closely with healthcare providers across Washington to identify the most common chronic conditions affecting patients and to create clear, accessible educational materials that support effective disease self-management. These chronic disease self-management plans are designed for use by providers in all healthcare disciplines and can be easily shared with patients to help them recognize symptoms, track changes, and understand when to seek medical care.
Each plan uses a simple, color-coded system to help patients monitor their health:
- Green flags show signs that a condition is stable and well-managed.
- Yellow flags highlight early warning symptoms that may require increased attention or a call to a healthcare provider.
- Red flags indicate emergency symptoms that need immediate medical care.
By giving these tools to your patients, you empower them to confidently identify changes in their chronic illness symptoms and take appropriate action—reducing preventable hospital visits and improving overall disease management.
