ALTC provides clients and families with information on programs and services, as well as suggesting alternatives which are designed to meet their specific needs.  The goal is to empower clients and families to plan choices before a crisis develops.

Case Management:   Case Management enables clients
                 to compare their needs with what is available in
                 their community.  


Referral & Accessing Programs: 
ALTC is an access point for obtaining information on services in your community that are available to seniors. Staff will provide information, services, referrals, assistance in accessing services, advocacy and screening to determine appropriate referrals or the need for Case Management services. We can help you find care providers in your community, or the type of Senior housing options that best meet your needs. We can help you access the following services and more:

SMART
Senior Medication Awareness Prescriptions Training
                 The SMART training program will help to
                 promote appropriate medication usage. 
                 Available in Yakima County.
