Referral and Accessing Programs
ALTC is an access point for obtaining information on services in your community that are available to seniors. Staff will provide information, services, referrals, assistance in accessing services, advocacy and screening to determine appropriate referrals or the need for Case Management services. We can help you find care providers in your community, or the type of Senior housing options that best meet your needs. We can help you access the following services and more:
HOME DELIVERED MEALS - (All Counties):
Providing nutritious
meals and other dietary services to homebound persons.
HEALTH SCREENING - (Franklin County):
Services are intended
to provide early detection of symptomatic diseases. Includes general health
assessment and appropriate referrals when abnormalities are detected.
FOOT CARE - (Asotin, Columbia, Franklin, Kittitas, Yakima Counties):
Services are intended to provide early detection of existing and potential
foot problems.
ADULT DAY CARE - (Benton, Franklin, Walla Walla, Yakima Counties):
Provides supervised activities in a group setting.
ADULT DAY HEALTH - (Benton, Franklin, Counties):
Services
may include rehabilitative nursing, health monitoring, Occupational Therapy,
personal care, social services and activity therapy provided in a group
setting.
TRANSPORTATION - (Asotin, Columbia, Franklin, Garfield, Kittitas,
Yakima Counties):
Provides rides to and from social services, medical
and health care services, meal programs, senior centers, shopping and other
activities.
S.M.A.R.T.
Senior Medication Awareness Prescriptions Training The SMART training program will help to promote appropriate medication usage. Available in Yakima County.
