ALTC provides clients and families with information on programs and services, as well as suggesting alternatives which are designed to meet their specific needs. The goal is to empower clients and families to plan choices before a crisis develops.
Case Management

Case Management assists vulnerable seniors in obtaining and effectively using support services. Case Management functions include a comprehensive needs assessment, service plan development, management and monitoring.
Supportive functions include advocacy, assistance, consultation, family support, crisis intervention and follow-up. Case Management enables clients to compare their needs with what is available in their community. As their needs and conditions change, appropriate changes can then be made to the services provided.
